Creation of a one-hundred item survey and custom cut score analysis database. The site will include the client’s logo and web site title as specified by the client. (add $400 for each additional group of 50 items).

 Includes:

1. Domain name (web address) specified by the client  (subject to availability).

2. 1 year hosting on a Window Server (host chosen by Michael H. Jones, Consulting).

3. ASP programming for database connection.

4. Delivery of an MSAccess database with a table containing all data.

5. A password protected web based site monitor that allows the client to monitor the progress of the Cut Score survey responses.

Up to four hours of hours of conference calls. Report of findings (20 to 30 pages), question ratings sorted by up to four geographic, biographic or demographic variables, bibliography.

Mail out of 1,000 survey notice post cards (based on a MSWord Mail Merge file supplied by the client).

Client requirements

1. Client provides a formated electronic MSWord mail merge file with all registrants.

2. Client must provide test items in final format.  No corrections will be made by the vendor after publication. All web connections will be 128 bit encryption. However, all security agreements with subject matter experts are the responsibility of the client and loss liability is assumed by the client.
